Impact Hoops 3x3 Basketball Tournament

By registering for the Impact Hoops One-Day 3x3 Basketball Tournament, I (the participant and/or parent/guardian of the minor participant) hereby acknowledge and agree to the following:

I understand that participation in this basketball tournament involves inherent risks of serious injury, including but not limited to sprains, fractures, concussions, bodily injury, and, in extreme cases, permanent disability or death. I voluntarily assume all risks associated with participation.

I, on behalf of myself, my child (if applicable), and our heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless Impact Hoops, Chadwick Wilkerson, the tournament staff, officials, volunteers, Michael L. Davis Gymnasium, and all affiliated parties from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death) that may occur during or as a result of participation in this event.

I further authorize any necessary medical treatment in the event of injury and agree to be financially responsible for any medical costs incurred.

I acknowledge that photographs and video recordings may be taken during the event and may be used by Impact Hoops for promotional, marketing, and educational purposes without compensation.

I certify that I am physically able to participate in this event and/or that I am authorized to register and act on behalf of the participant(s) listed in this registration.

I have read this waiver, fully understand its terms, understand that I am giving up substantial legal rights by agreeing to it, and agree to its conditions voluntarily and without inducement.
